(Due to State & Local requirements if you have more than one child a separate form must be filled out for each child.)
* It is required that this form be received by the school prior to the first day of school
** It is required by law that we have up-to-date immunization records on file for all of our students. Therefore all Kindergarteners and all of the students new to Lerner must submit this information to us as soon as possible.
